Please Note: Receipt of this form does not confirm the booking. The booking will still need to be confirmed by the Health and Wellbeing Team Leader.

Hours of operation

Monday - Thursday: 6am to 9pm
Friday: 6am to 8pm
Weekends: 8.30am to 5pm
Public Holidays: 8.30am to 5pm
